It is important to get permission when you are writing someone else’s story so all biographical entries need to be accompanied by a form authorising you to tell their story. Click on the link below, print out the form, fill in your name and have the subject of your story sign and date it. If the subject is no longer alive then their next of kin should sign the form. Scan in the file, upload it and submit it along with your entry. If you don’t have access to a scanner, an image taken on a smart phone can work just as well.
You do not need to upload the Biography Consent Form with your story. We will only request it if we deem it necessary.
